We use Websense (recently renamed Forcepoint) as our content filter. It's very expensive and infuriatingly complicated, but it's also extremely effective. They categorize proxies which you can then block, and even something simple like blocking the word 'proxy' in the URL has helped tremendously. I don't want to say it's 100% since I'm sure there's some way around it, but we have yet to have a problem.
