Couple of things.
Are you in the right subscription in Azure powershell?
Get-AzureSubscription
Have you the right storage account?
Get-AzureStorageAccount | Format-Table -Property Label
Have you set the right storage container?
Get-AzureStorageContainer


Try
Add-AzureVhd -LocalFilePath "C:\tester.vhd" -Destination "https://tester******.blob.core.windows.net/vhd/tester.vhd"

So I deleted everything I had before and started fresh ..................  WTF

Have you tried using the ARM cmdlet? 
Add-AzureRmVhd

Hey Pieter "AzureRmVhd RM"  What is the RM ..?  I do not see that as one of the Alias's.

AzureRM followed with the rest are the Azure Powershell Commands to allows you manage Azure Resource Manager based resources.
The AzureVHD command is for the Classic Azure IaaS

Not confusing at all. Not even a little.
Makes perfect sense.

The drama continues.  As you can tell I'm learning as I'm going so I apologize for the ignorant questions and frustration.

 As I stated before the vhd is uploaded.

I believe the next step is to add the new image to the list of custom images.  So I did the following and it's not happy with what I'm doing grrrrrrrrrrrrrrrr
